NC 1504-1.10 Model Forms

- The SEA has developed model forms to assist parents and other parties in filing a State complaint under NC 1501-10.1 through NC 1501-10.3.
- Use of the model form is not required.
- Parents, LEAs, and other parties may use the appropriate model form described in paragraph (a) of this section, or another form or other document, so long as the form or document that is used meets, as appropriate, the content requirements in NC 1504-1.9(b) for filing a petition for a due process hearing, or the requirements in NC 1501-10.3(b) for filing a State complaint.
